All SDA students are encouraged to try out for a sports team.\u00a0 In order to try out, the athlete must complete the following clearance process.\u00a0\u00a0<\/span>Click here<\/span><\/a>\u00a0for instructions on using homecampus.com<\/span><\/p>\n

  1. \n

    Go to\u00a0<\/span>www.homecampus.com<\/span><\/a>\u00a0and complete the online clearance for the 2025-26 school year (do not use last year’s clearance).\u00a0<\/span><\/p>\n<\/li>\n

  2. \n

    Submit a picture of health insurance card either via email to\u00a0<\/span>megan.thompson@sduhsd.net<\/span><\/a>\u00a0or drop off at the Athletic Office.<\/span><\/p>\n<\/li>\n

  3. \n

    Submit a physical performed by an MD, DO, PA, or NP.\u00a0 The physical must not be older than one calendar year and must include the provider’s signature on this form:\u00a0\u00a0<\/span>Download Physical Form Here<\/span><\/a>.\u00a0 Only the Medical Eligibility Form (page 4) should be returned to the school site.<\/span><\/p>\n<\/li>\n

  4. \n

    Use of Private Automobile Form<\/span><\/a>.\u00a0 All athletes and parents who drive when District transportation cannot be utilized must submit a form in their name.<\/span><\/p>\n<\/li>\n<\/ol>\n

    Insurance<\/span><\/strong><\/p>\n

    The San Dieguito Union High School District does not provide medical insurance coverage should an athlete get hurt during a school activity.\u00a0 However, the District does make student-accident insurance available if the athlete does not have insurance. \u00a0 Please visit\u00a0<\/span>www.peinsurance.com<\/span><\/a><\/p>\n

    \n

    PE Credit for Athletics Option<\/span><\/strong>\u00a0\u00a0\u00a0<\/span><\/p>\n

    PE Credit for Athletics applications will not be accepted until the start of athletic activities.\u00a0\u00a0<\/span>The link for the a<\/span>pplication for the 2<\/span>025-26 school year will be posted prior to Fall sports starting.<\/span><\/p>\n

    A student can earn 5 PE credits per season with a maximum of 20 credits earned through participation on a SDA sports team.\u00a0 The option is available to grades 10-12.\u00a0 \u00a0 Credits will\u00a0<\/span>not\u00a0<\/span>be given retroactively.\u00a0\u00a0<\/span><\/p>\n
