ATHLETIC CLEARANCE
Athletes will not be allowed to tryout without a completed athletic clearance.
Please make sure to complete your athlete’s clearance process by July 31, 2026. If your daughter received clearance for another sport, you must still revisit the clearance website and add Girls Flag Football to the list of sports for her to be cleared.
- Go to www.homecampus.com and complete the online clearance for the 2026-27 school year (do not use last year’s clearance).
- Submit a picture of your health insurance card either via email to megan.thompson@sduhsd.net or drop off at the Athletic Office.
- Submit a physical performed by an MD, DO, PA, or NP. The physical must not be older than one calendar year and must include the provider’s signature on this form: download physical form here
- Only the Medical Eligibility Form (page 4) should be returned to the school site.
Donations
Fundraising helps promote the growth and success of Girls Flag Football at SDA. Donations are used to help purchase team uniforms, team swag, equipment, stats tracking software, team photos, team bonding events, tournaments…and much more! To cover these expenses, we are requesting a voluntary donation of $250 per player.

Cash donations can be made to the Athletic Council of the SDA Foundation.
