 ***Players must have Athletic Clearance to participate in tryouts.***

TRANSPORTATION FORMS:
The District provides a bus for away games; however, sometimes no bus is available, and we need to drive players. For this situation, we need permission forms filled out in advance.  →

Field Trip Form=Permission for the athlete to go to the event in a private vehicle.
Use of Private Auto form= Permission for parent or your athlete to drive others to games.
